Introduction. Treatment of hilar cholangiocarcinoma remains one the most complicated issues surgical hepatology. As known, is resistant to chemotherapy and radiation therapy, only a option an effective way improve patient survival.The aim study was clinical outcomes patients with by evaluating efficiency various options for resection reconstructive restorative stages treatment.Methods. The involved findings 68 cholangiocarcinoma. Biliary decompression performed in 61 (89.7%) cases. Radical interventions were 35 (51.4%) cases, palliative 33 (48.6%) cases.Results. Specific complications at stage biliary noted 4 (6.5%) inadequate drainage tract occurred 5 (8.2%) syndrome "rapid tract" recorded 14 (23.0%) cholangitis - 12 (19.7%). Intraoperative 2 (15.3%) postoperative 6 (17.1%) Mortality rate 8.5%. In long-term period, stenosis biliodigistic anastomosis 11.4% tumor recurrence 28.6% five-year survival 17.0%.Conclusion. Resection bile ducts combined hemihepatectomy operation accompanied high risks, especially height jaundice elderly patients. Performing sparing hiatal liver provides acceptable rate. proposed method form terms preventing insolvency anastomosis.
